Excess Skin Removal Surgery

Excess skin removal surgery covers a range of surgical procedures designed to remove redundant skin, improve body contour, and allow patients to fully realise the results of their weight loss.

Losing a significant amount of weight is a considerable achievement. However, for many patients, dramatic weight loss results in loose, excess skin that cannot be addressed through lifestyle changes alone.

Here at Manchester Plastic Surgery, we know that every patient's journey is different. Our consultant plastic surgeons take the time to understand your individual circumstances, your goals, and your concerns before recommending any surgical approach. This page will walk you through the surgical options available, what to consider before proceeding, and how to know whether you might be a suitable candidate.

What Is Excess Skin Removal Surgery?

When the body gains weight, the skin stretches to accommodate the increased volume of fat tissue beneath it. Over time, and particularly after significant or sustained weight gain, the skin loses much of its natural elasticity. The collagen and elastin fibres that give skin its ability to contract are damaged or depleted, meaning that when weight is lost, the skin is unable to retract to fit the body's new, smaller frame.

This results in loose, sagging skin that can affect multiple areas of the body, including the abdomen, arms, thighs, breasts, back, and neck. In many cases this excess skin causes not only cosmetic concerns, but physical discomfort, including skin irritation, rashes, hygiene difficulties, and restricted movement.

Excess skin removal surgery, also referred to as post weight loss surgery or loose skin removal surgery, is often the only clinically effective way to address this issue. Diet, exercise, and non-surgical skin tightening treatments cannot remove excess skin once it has lost its elasticity. Surgery removes the redundant skin, tightens the underlying tissues, and reshapes the body to better reflect the patient's weight loss.

In many cases, more than one procedure will be required, either performed simultaneously or in a staged approach, depending on the extent of correction needed and the patient's overall health.

Surgical Options for Excess Skin Removal

Choosing the right procedure will depend on your personal journey, how much excess skin you have and which part of your body is affected most.We offer lots of options to help you complete your weight loss journey at MPS and here we have pulled together a list of procedures that you might be interested in below.

Tummy Tuck (Abdominoplasty)

The abdomen is one of the most commonly affected areas following significant weight loss. A tummy tuck, also known as abdominoplasty, removes excess skin and fat from the abdominal area and tightens the underlying abdominal wall muscles, resulting in a flatter, more contoured midsection.

For patients who have lost a large amount of weight, an extended abdominoplasty may be recommended, which addresses excess skin extending around the flanks and lower back in addition to the abdomen.

Breast Lift (Mastopexy)

Significant weight loss frequently causes the breasts to deflate and sag, as the skin and breast tissue are no longer supported by the volume of fat that was previously present. A breast lift, or mastopexy, removes excess skin, reshapes the breast tissue, and repositions the nipple to a more natural height, restoring a firmer and more lifted breast profile.

In some cases, a breast lift may be combined with breast implants to restore lost volume at the same time.

Arm Lift (Brachioplasty)

Excess skin on the upper arms, commonly referred to as "bingo wings", is a frequent concern for patients following major weight loss. An arm lift, or brachioplasty, removes this loose, hanging skin from the upper arm, restoring a firmer and more defined arm contour. The procedure involves an incision along the inner or underside of the upper arm, and in some cases may be combined with liposuction.

Thigh Lift

Loose skin on the inner or outer thighs can cause significant discomfort, including chafing and restricted movement, as well as affecting the fit of clothing. A thigh lift removes this excess skin and tightens the surrounding tissue, improving the contour and proportion of the legs. The procedure can target the inner thighs, outer thighs, or both, depending on the patient's needs.

Back Lift

Significant weight loss can leave excess skin and fat along the back and flanks, resulting in rolls or folds of skin that are difficult to conceal with clothing. A back lift removes this excess tissue, often in combination with liposuction, to create a smoother and more contoured back profile. This procedure is sometimes performed as part of a broader body lift approach.

Gynecomastia Surgery (Male Chest Reduction)

Men who have lost a significant amount of weight may be left with excess skin and tissue on the chest. Gynecomastia surgery removes this excess skin and chest tissue, creating a flatter, firmer chest profile. This procedure is also appropriate for men who have developed enlarged breast tissue as a result of hormonal changes associated with obesity.

Facial Rejuvenation After Weight Loss

Significant weight loss does not only affect the body, it can also have a notable impact on the face and neck. As fat is lost from the facial area, skin that was once supported by the underlying tissue can become loose and deflated, often accelerating the appearance of ageing. If you have noticed these changes in the mirror, you are not alone, and there are surgical options that can help.

Neck Lift

Weight loss can cause the skin of the neck and jawline to become lax and saggy, resulting in a loss of definition in this area. A neck lift addresses this by tightening the underlying muscles and removing excess skin, restoring a more defined and youthful neck contour. It is commonly performed in conjunction with a facelift for patients with more extensive facial changes following weight loss.

Facelift

Dramatic weight loss can accelerate the appearance of facial ageing, causing the skin of the face, cheeks, and jawline to appear hollow or deflated. A facelift addresses this by lifting and tightening the underlying facial tissues and removing excess skin, restoring more natural and proportionate facial contours.

Am I Suitable for Excess Skin Removal Surgery?

Excess skin removal surgery is a significant undertaking and patient suitability must be assessed carefully before proceeding. We consider lots of different things when deciding if patients are suitable and although every case is different, here are the main areas we look at:

Weight Stability

One of the most important requirements before undergoing excess skin removal surgery is that your weight has been stable for a sustained period, typically a minimum of six to twelve months. If you are still actively losing weight, surgery should be postponed. Undergoing surgery before your weight has stabilised increases the risk of complications and may compromise your results, as continued weight loss following surgery can lead to further skin laxity.

If your weight loss has been achieved through GLP-1 medications such as Ozempic or Wegovy, it is particularly important that your dose has been stable and your weight has plateaued before seeking a surgical consultation.

BMI

While there is no universal BMI threshold for excess skin removal surgery, your body mass index is an important factor in assessing surgical risk. Patients with a higher BMI face an increased risk of surgical complications including infection, wound healing difficulties, and deep vein thrombosis. In general, patients with a BMI below 30 are considered lower risk. Your surgeon will assess your BMI alongside other health factors during your consultation and advise accordingly.

General Physical Health

Excess skin removal surgery is performed under general anaesthetic and, in many cases, involves a significant degree of surgical correction. Patients must be in good general health prior to surgery. Certain medical conditions, including uncontrolled diabetes, cardiovascular disease, and blood clotting disorders, can increase the risks associated with surgery and may affect your eligibility. A thorough medical assessment will be carried out as part of the pre-operative process.

Psychological Readiness

It is important that patients approach excess skin removal surgery with realistic expectations about what can be achieved. Surgery will improve the contour of the body and remove redundant skin, but it will result in scarring, and results will vary between individuals depending on skin quality, age, and the extent of correction required. Patients should feel psychologically prepared for the surgical process and the recovery period, and should be pursuing surgery for personal reasons rather than as a result of external pressure.

Smoking

Smoking significantly increases the risk of surgical complications, particularly in relation to wound healing and tissue perfusion. Patients who smoke will be required to stop smoking for a minimum period before and after surgery, typically at least six weeks prior to the procedure. Your surgeon will provide specific guidance on this during your consultation. Patients who are unable or unwilling to stop smoking may not be suitable candidates for surgery.

Frequently Asked Questions

How do you remove excess skin after weight loss?

Many patients arrive at this point having already explored exercise as a solution. Weight training can increase muscle mass, which helps to fill out the skin and reduce the visible appearance of looseness, and it is always worth pursuing for overall health. However, after significant or rapid weight loss, the skin loses its elasticity and is simply unable to retract on its own. Non-surgical treatments cannot remove excess skin either. Surgery remains the only clinically effective solution, and the right procedure will depend on which areas of the body are most affected, whether that is the abdomen, arms, thighs, breasts, neck, or a combination.

Can loose skin be removed without surgery?

Non-surgical treatments such as radiofrequency therapy and laser skin tightening can improve skin tone and laxity to a modest degree, but they are not able to remove excess skin. For patients with significant loose skin following major weight loss, surgical intervention is the only effective solution. A consultation with a consultant plastic surgeon will help determine the most appropriate approach for your individual circumstances.

How long after weight loss can I have excess skin removal surgery?

Most surgeons recommend waiting until your weight has been stable for a minimum of six to twelve months before proceeding with excess skin removal surgery. This allows the skin to retract as much as it naturally will and gives your surgeon an accurate assessment of how much excess tissue needs to be removed. Proceeding too early, before weight has fully stabilised, increases the risk of requiring further surgery if additional weight loss occurs.

Can multiple excess skin removal procedures be performed at the same time?

In some cases, yes. Certain procedures can be safely combined in a single surgical session, which has the advantage of reducing the total number of anaesthetics required and consolidating the overall recovery period. Common combinations include abdominoplasty with breast lift, or arm lift with thigh lift. However, combining procedures increases the overall length and complexity of surgery, and your surgeon will always prioritise patient safety when making recommendations. In cases where a significant degree of correction is needed across multiple areas, a staged approach, with procedures performed separately over time, may be recommended.

Will I have scars after excess skin removal surgery?

All excess skin removal procedures involve surgical incisions and will result in permanent scarring. Your surgeon will aim to place incisions in locations that can be concealed beneath clothing and swimwear where possible, and will discuss scar placement with you in detail during your consultation. Scars will typically appear red and raised initially, before gradually fading over the course of twelve to eighteen months. Whilst scars will not disappear completely, most patients find that the improvement in body contour significantly outweighs the presence of surgical scars.
